my husband has been given orders to japan and is now in training for his new MOS. i just had a baby and now need to add him to the levy DA 5888. i have no idea where to go or what i will need for that matter for this i am on Ft.Bragg. and have alot of clearing for this family to do including going to transportation and finding a place on post. getting passports and so on. can anyone help.

Okay, I am somewhat confused as to what a Levy DA 5888. But after looking it up, you are saying that you need to add your baby to the form DA 5888. Which is the EFMP overseas screening application.

Have you scheduled an appointment with the EFMP clinic there at Ft. Bragg? I am looking at the FT. Bragg website and it states that the soldier or the spouse must initiate the overseas screening appointments with the EFMP Clinic.

If you contact the EFMP clinic and inform them that you just had the baby and if you could add your baby's name to the screening.

I'm surprised no one told you what to do with that form.

i need to add him to the da 5888 form first EFMP doesnt do that but i think i figured it out i am going to go to the soldier support center today... thank you so much for your reply and God Bless

yes i have thank you deers then tricare then da 5888 and now the EFMP this is alot to keep up with lol i dont know how some people do it
