I am posting this in the MK Forum since most of the unit's I've been to have MK's as their vehicle managers.
Does anyone know where I can find the actual GV / Forklift operators licenses? My unit Safety Officer was asking. I've looked on the MLC website, but I'm having a tough time. Also, do we need to send someone to a 'train the trainer' class in order to issue them? Thanks for your help.
The paper licenses can be ordered from gsaadvantage its OF-346. I can't remember if there's a hyphen in there or not. You'll need a typewriter to fill these out. If you need them quick you can look in the online forms database and get them from there too.
Kevin ya may want to check with Dist. or MLC in the area your in- way back I know they were not required in some dist. but that may have changed. the mlc vehicle section even has the train the trainer clases. and if still needed the trainer/signer of the card has to be designated as qualified to train people.
I actually went to a seebee License Instructor course out in Mississippi. The course basically taught me the ins and outs of the paperwork and how to create Govt DL folders. Basically you have your application NAVFAC 11240/10, Medical questionair OF-345, Privacy Act Statements...ect. I am also in the process of designing an Access database for issue and tracking of Licenses. Just need to finish up a few things with the switchboard and create a job aid for certain info you will have to change to tailor the database to your unit. I am going to forward the database to the DOG and perhaps the MLC's (if I can find a POC) for distribution.
According to the current edition of the Motor Vehicle Manual, licenses are required for SPME such as tiger carts, golf carts, aircraft tugs and any other offroad vehicles. Further, they are required for GVs other than normal passenger travel such as: trucks with plows, that pull trailers, and things of that nature. Of course we are all aware of the need to have one for forklifts. This is required for all personnel that operate any of the above equipment. You are allowed to grandfather in individuals that are already using the equipment, but a training program such as check rides has to be established to deal with newly reporting personnel.
