I have looked in the front of my medical record and have found that one Anthrax vaccine is do***ented. I believe that I may have a had a second vaccine. Is there a medical deck log that the ship would have maintained that would include these records of when they gave the vaccine and who it was given to including the lot #. Is this available through FOIA or some other means? This would have been given in 2000.

IF it wasn't listed in the DD 2766 at the beginning of section one of your medical record, perhaps it is on the shot record which is a few pages behind it. Some people don't update BOTH and just put it in one or the other, which is irritating.

But, if they didn't put it into your hard copy record, perhaps it will show up in your IMR or MRRS (depending on where you're at) and it can be retrieved from one of those databases which your readiness data is maintained. If you're transferring from one ship to another, your SAMS data is available electronically and if it is in there, you'll be good to go.

But, as all things go - no documentation means it never happened. Medical is responsible for medical data.
