Nearly every Department of Veteran's Affairs facility has a booklet entitled 'Patient and Nursing Home Resident Rights and Responsibilities.'

It has four sections to it. They are:

1. Respect and Nondiscrimination.
2. Information Disclosure and Confidentiality.
3. Participation in Treatment Decisions.
4. Issues or Concerns

Any concerns about patient care and safety not resolved by the hospital can be reported to Joint Commission.

Phone is 1-800-994-6610, Fax: (630) 792-5636

Email is complaint@jointcommission.org
Mail: Office of Quality Monitoring, Joint Commission, One Renaissance Boulevard, Oakbrook Terrace, IL 60101.

For further information on patient rights and responsibilities, you may contact the patient advocate at your local facility. Suggestions improve VA care. Service is important to VA facilities and compliments are welcomed.

Suggest that if you are visiting a VA facility, pick up a copy of the booklet so that you can be better informed as to your rights and responsibilities as a veteran.

Thanks.
