For those of you who are fairly new to the VA, there is a single form you can request that provides a wealth of information on your status within the system. It is called a HINQ (Hospital INQuiry)report, it gives you a listing of all service connected conditions as well as the effective dates for each as well as most information on your status on co pay's In addition to a complete list and status of each disability granted, it also contained an entry stating "Perm., Total Disability = Yes". I request one everytime there is a change in my rating.

The process to get it is quite simple, Go to freedom of Information office tell them what you want and sign the release form. It is usually 2 pages or less so there should be no reason they cannot simply hand it to you from the printer and send you out a happy veteran who got what they wanted
